complained a single man in his early 20s是修饰谁的---> this is the subject, or main clause.
为什么a single man 放在 后面是不 是应该有个by  ===> NO
Answer:
 A simple sentence contains a single subject and predicate. It describes only one thing and has only one verb.
Subject - verb -  predicate

Subject= A (single) man (in his early 20s)
verb = complained
Predicate = (in reality, however,) single youths (who are also sexually active are not well covered by (government-sponsored sexual and reproductive) health services (like consultations, health checks and free contraceptives like condoms).  Take out all the adjectives: a simple sentence emerges:

A man complained (that) single youths are not well covered by health services.
Or:
Single youths are not well covered by health services, a man complained.
Or:
Single youths are not well covered by health services, complained a man.
 the word “by” is not needed.

no,

because there is nothing that is "emphasized" here.
Placement of the subject and verb does not make it an inverted sentence.
inversion is used to emphasize something and usually it is used with other adjectives, like seldom, or "should it be.."
It so happened this is a simple sentence which is very long and has many modifiers.

yes for both cases, grammatically, the word "that" is implied and hidden.
For readability sake, the second case should start with the word "that" :
--That, in reality, however, single youths who are also sexually active are not well covered by government-sponsored sexual and reproductive health services like consultations, health checks and free contraceptives like condoms,  a single man in his early 20s complained.

The first case, the word "that" is surely implied-- as marked off by a comma after the word condoms, and started with the verb complained followed by the subject.
--(That,) in reality, however, single youths who are also sexually active are not well covered by government-sponsored sexual and reproductive health services like consultations, health checks and free contraceptives like condoms, complained a single man in his early 20s.
